AUTO INSURANCE REQUEST FORM

Auto insurance is mandatory for all UC owned vehicle. All autos must be scheduled with Risk Services. It is the department’s responsibility to submit the Auto Insurance Request Form to Risk Services to obtain insurance for all autos assigned to that department. If an auto is involved in a loss prior to notifying Risk Services, funding for that loss may be denied.
